Lost, Stolen, or Destroyed Certificates

If your RN certificate was lost, stolen, or destroyed, you may purchase a replacement from the Board online through BreEZe Online Services or by mailing a completed Request for Duplicate Certificate form and payment to the Board at the following address:

NOTE: The Board will not waive the duplicate certificate fee if your certificate was not received as a result of a change in address of record or if the Board is not notified within 90 days of issuance.

Name Change

If you would like a replacement wall certificate as a result of a name change, you must provide a completed Request for Duplicate Certificate form, including required fee and legal documentation, and submit to the Board of Registered Nursing at the following address:
